Copyright Notice Procedure
Understanding and respecting copyright laws are crucial aspects of operating within the digital landscape. At ND Forms PDF, we prioritize the protection of intellectual property and commit ourselves to addressing copyright infringement issues swiftly and effectively. Through our Copyright Notice Procedure, we ensure that rights owners can easily report instances of alleged infringement, enabling us to promptly respond to and resolve such concerns. This page outlines the steps necessary to file a copyright notice with ND Forms PDF and provides essential information for both rights owners and those who believe their content has been wrongly identified.
Filing a Copyright Notice
To file a copyright infringement notice, rights owners or their authorized agents should prepare a written notification that includes the following information:
- A physical or electronic signature of the copyright owner or a person authorized to act on their behalf;
- Identification of the copyrighted work claimed to have been infringed;
- Identification of the material that is claimed to be infringing, with sufficient detail so that we may locate it on our site;
- Contact information where we can reach the complainant, including name, address, telephone number, and, if available, an email address;
- A statement that the complainant believes in good faith that use of the copyrighted material in the manner complained of is not authorized by the copyright owner, its agent, or the law; and
- A statement that the information in the notification is accurate, and under penalty of perjury, that the complainant is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
All copyright notices must be sent to our designated agent at dmca@ndformspdf.com.
Response to Copyright Notices
Upon receipt of a valid copyright notice, ND Forms PDF will take immediate action, which may include removal of the allegedly infringing material from the website. We will also notify the individual or entity that posted the material, providing them with an opportunity to respond with a counter-notice should they dispute the claim of infringement.
Filing a Counter-Notice
If you believe that your content was wrongly removed due to a mistake or misidentification, you can send us a counter-notice. Similar to filing a copyright notice, the counter-notice must include the following:
- Your physical or electronic signature;
- Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access to it was disabled;
- A statement under penalty of perjury that you believe in good faith that the material was removed or disabled as a result of mistake or misidentification;
- Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial district in which ND Forms PDF may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
Send your counter-notice to our designated agent at dmca@ndformspdf.com. We will review your submission and respond accordingly.
