Filing a Copyright Notice

Response to Copyright Notices

Upon receipt of a valid copyright notice, ND Forms PDF will take immediate action, which may include removal of the allegedly infringing material from the website. We will also notify the individual or entity that posted the material, providing them with an opportunity to respond with a counter-notice should they dispute the claim of infringement.

Filing a Counter-Notice

If you believe that your content was wrongly removed due to a mistake or misidentification, you can send us a counter-notice. Similar to filing a copyright notice, the counter-notice must include the following:

  • Your physical or electronic signature;

  • Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access to it was disabled;

  • A statement under penalty of perjury that you believe in good faith that the material was removed or disabled as a result of mistake or misidentification;

  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial district in which ND Forms PDF may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
